Patient Information

We are inside the Eastlake Medical Building at the corner of Garland Road and Centerville. Our office is on the northwest side of the building.

Please note that we have recently relocated within the building – our new office is in Suite 210 on the second floor.  Patients can take either of the two staircases in the atrium to the second level or use the elevator which is located on the north side of the atrium in its center.
